Duties and Responsibilities:
Patient Assessment and Counseling:
- Conduct thorough assessments of patients' social, emotional, and financial needs.
- Provide individual and family counseling to help patients cope with the emotional aspects of illness, hospitalization, and treatment.
Discharge Planning:
- Collaborate with medical staff to develop and implement discharge plans that ensure patients have the necessary support and resources when they leave the hospital.
- Coordinate with local government agencies and other external resources to facilitate a smooth transition from hospital to home or another care facility.
- Educate patients and their families about available resources, including financial assistance, home care options, and support groups.
Advocacy and Support:
- Advocate for patients' rights and needs within the healthcare system, ensuring they receive appropriate care and services.
- Assist patients and families in navigating complex healthcare systems, including insurance issues, legal matters, and eligibility for public assistance programs.
Community Resource Coordination:
- Maintain an up-to-date knowledge of community resources, including financial assistance, and social services, and connect patients with these resources as needed.
- Develop and maintain relationships with community organizations, government agencies, and other stakeholders to enhance resource availability for patients.
Documentation and Reporting:
- Maintain accurate and timely documentation of all patient interactions, assessments, and interventions in compliance with hospital policies and legal requirements.
- Prepare reports and case summaries as required by the hospital administration or external agencies.
- Collaboration and Teamwork:
- Participate in multidisciplinary team meetings, case conferences, and other collaborative efforts to ensure comprehensive patient care.
- Provide consultation and support to healthcare staff regarding the social and emotional aspects of patient care.
- Perform other tasks as assigned.
Qualifications:
- Bachelor’s degree in Social Work (BSW)
- Must be a licensed Social Worker (LCSW, RSW, or equivalent as required by local regulation).
- At least 1–2 years of relevant experience in social work or case management.
- Excellent communication and interpersonal skills.
- Strong empathy, patience, and active listening skills.
- Knowledge of community resources, government aid programs, and social services.
- Ability to handle sensitive and confidential information professionally.
- Skilled in documentation, report writing, and case follow-ups.
- Capacity to work independently and collaboratively in a team setting.
- Willingness to work flexible hours and in field/community settings as needed.

Pay: Php14,600.00 - Php20,000.00 per month
