I. Duties and Responsibilities:
1. Provides social work services to inpatients, outpatients and hospital personnel.
2. Provide discharge planning assistance for inpatients and outpatients as deemed necessary and as requested.
3. Coordinate coverage to ensure that continuity of care needs of all patients is met in all service areas.
4. Prepares referral slip to PCSO.
5. Perform social casework functions in patients with social emotional and/or environmental problems that may affect their medical situation through purposeful interview with them and their families.
6. Perform eligibility study on patients with social and financial capabilities.
7. Maintains quality results by following standards.
8. Protect hospital’s value by keeping collection information confidential.
9. Perform such other duties and responsibilities analogous or related to and or implied from the above enumeration of duties and responsibilities

- 8 hour shift
