Position: Scheduler & Patient Coordinator

We specialize in delivering personalized care through Nurse Practitioners and other healthcare providers in the comfort of patients' homes. We are seeking a highly organized and compassionate Scheduler & Patient Coordinator to join our growing team.
Responsibilities:
- Schedule appointments for patient visits in alignment with the provider’s availability and patient needs.
- Coordinate with healthcare providers to ensure timely and efficient patient care.
- Manage patient intake, ensuring accurate patient information is captured and processed.
- Maintain communication with patients regarding appointment reminders and any necessary pre-visit instructions.
- Ensure accurate and timely documentation in our electronic health records (EHR) system.
- Act as a liaison between patients and the medical team to ensure optimal patient care.
- Assist with patient follow-up and track ongoing care needs.
Qualifications:
- Experience in scheduling, patient coordination, or customer service in a healthcare setting preferred.
- Strong organizational and multitasking abilities.
- Proficiency in Microsoft Office and familiarity with EHR systems.
- Excellent communication skills and attention to detail.
- Ability to work independently and as part of a team.
Job Types: Full-time, Part-time
Pay: Php25,000.00 - Php45,000.00 per month
